Why They're dirty:
The song contains somewhat difficult-to-comprehend lyrics, but, like several other songs on the album, they are full of sexual innuendo. In this case, "Custard Pie" refers to a woman's sexual organs and the song is rife with references to oral sex.

I don't think I can honestly claim that the dirtiness of these lyrics initially escaped me. When I submit them all the same it is because I find this to be one of the truly great metaphors for lovemaking in a rock (blues) song....
