Is it bad that I still remember someone like Juan Acevedo? In 2003 the Yankees were plagued by the lack of quality middle relievers. Granted their clutch offense were bad as well but the bullpen with relievers not named Mariano Rivera have been among the main weakness, the weakness along with terrible RISP production from the offense what would led to the undoing in the World Series that year.

Juan Acevedo was signed after the closer Mariano Rivera got hurt so for a while he filled in as a closer. Granted he was shaky but he was able to go 6 for 6 on save opportunities. But in the end he can't be trusted to hold the lead even if his life depended on it. Was also responsible for delaying a certain juiced up starting pitcher's quest for 300 wins twice, most notably giving up that home run against the Cubs to blow the lead.

Eventually he was released by the team where he would sign with the Blue Jays before being let go as well. He retired after the season.

Granted Antonio "Fatsuna" Osuna and Chris "Hemlock" Hammond aren't any better but at least they did better job than Acevedo did. Osuna was actually decent for a while until he got hurt mid-season, after that he wasn't the same.

However these guys aren't the ones that ticked me off the most, the one who did that is a certain someone who happens to be related to a good pitcher with the Angels...forgot his name, gave up Ted Lilly for that guy.
